     All on the job injuries must be reported immediately, or within 24 hours, to your immediate supervisor or a supervisor at the time of injury.  If you are a campus employee, please also be sure to report your injury to the campus nurse so the extent of your injury can be assessed.

    The Employee Injury Report and the Supervisor Accident Investigation Report must be completed and submitted to Kimberly Heiskell within 24 hours of the injury.

    All reports must be sent to Kimberly Heiskell, Coordinator of Risk Management, via email at kheiskell@ems-isd.net or via Fax at 817-232-3596.
